PBO REGISTRATION DISCLAIMER Upon deposit of the registration fee, Palos Baseball Organization, Inc. agrees to provide my child the opportunity to play organized baseball during the 2024 Palos Baseball season. Having been informed of the intent of Palos Baseball Organization, Inc. to provide supervised baseball games for youth, and that the participation in baseball may result serious injuries and protective equipment does not prevent all injuries to players, 

I/we, the parent(s) or guardian of the child, hereby gives permission for his/her participation in any and all activities, and I/we assume all risks and hazards incidental to the conduct of activities, including transportation to and from activities during the 2024 Palos Baseball season. 

I/We further hereby release, absolve, indemnify and hold harmless Palos Baseball Organization, Inc., organizers, sponsors, trustees, officers, managers, and supervisors or any other supervisors appointed by them. I/We likewise release from liability any person transporting my/our child to or from activities of the organization. I/We also certify that my/our son/daughter is medically fit to participate in baseball activities and grant permission for the Directors, Trustees, Managers, and/or Coaches to act in their best judgment in any emergency requiring medical attention. I/we have been informed and agree with the refund policy for the 2024 Palos Baseball Organization registration fees. For each child registered for the 2024 PBO season that tries out and makes a 2024 PBO Travel team (name posted on the PBO Website), the registration fee is non-refundable as of the posting.

For each child registered for the 2024 PBO season that is not playing on a 2024 PBO Travel team for the season, the registration fee including the Field Days, Snack Fee and concession buy out is refundable until December 31st prior to the season. For each child registered for the 2024 PBO season that is not playing on a 2024 PBO Travel team for the season, starting January 1st, upon written request, I/we will receive a prorated field day, snack fee and concession buy out. After January 31st, the total fees are non-refundable. 

I/we understand that I/we will be charged a onetime $45 snack fee, a $50 refundable concession but out as well as a $50 refundable field day for the family and not per player for the 2024 Palos Baseball season. This fee will be refunded to me/us after I/we have worked a field clean up day, a concession stand 4-hour shift or secure a NEW $500 sponsor. If I/we do not participate in a field clean up day, concession 4-hour shift or secure a sponsor as listed on page 1, I/we will forfeit my/our field day and concession fees and Palos Baseball Organization, Inc. shall use these funds as Palos Baseball Organization, Inc. deems necessary to run the league. I/we also understand that for each child playing.

Palos Baseball Organization, Inc. reserves the right to cancel the registration of any player participating in any sport which interferes or conflicts with regular season and playoff baseball games. Further, Palos Baseball Organization, Inc. specifically prohibits any player from playing organized baseball at any other club, team, group, organization, or league etc. and if such occurs the player shall be subject to cancellation of the registration and removal from Palos Baseball Organization, Inc. Palos Baseball Organization, Inc. reserves the right to suspend or expel any player, coach or manager for conduct detrimental to the league pursuant to the provisions of the Palos Baseball Organization, Inc. rules and regulations. 

I/we understand that by showing interest to be a manager or assistant coach in the 'OTHER' section of the Registration Form does not guarantee me/us to be a manager or assistant coach. If I or the other parent/guardian listed on this form is selected as a volunteer manager, I/we agree to participate in all training, educational, and informational manager’s meetings made available by Palos Baseball Organization, Inc., including but not limited to, becoming a certified coach through a training company provided by or acceptable by Palos Baseball Organization, Inc. and further agree and understand that such are required by Palos Baseball Organization, Inc. to fulfill its obligation with its affiliation with the City of Palos Heights. If selected to be a manager or coach, I/we also hereby give our permission and consent and agree to provide any and all information necessary to Palos Baseball Organization, Inc. to obtain a full background check for the purpose of selecting me/us as a manager or coach. 

I/we understand that by showing interest to be an umpire or part of the field crew in the 'OTHER' section of the Registration Form does not guarantee me/us to be an umpire or a field crew member.
